Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.

Referral and Insurance Authorization:
Some insurance providers mandate a referral from a primary care physician before scheduling. Without this referral, patients may need to assume financial responsibility for the visit. If your appointment requires authorization from your insurance provider (such as an HMO plan), please contact us directly before scheduling.
